Refer clients to other supportive services as needed, including food pantries, clothing closets, legal aid services, substance abuse and mental health services; coordinating these services as needed to obtain wrap-around support for clients. Connecting clients to other LCN services, such as technology training, food programming, and youth services. Provide budgeting and financial counseling to assist clients in becoming financially self-sufficient and in assuming responsibility for housing costs.

Provide counseling and life skills training to assist clients in making positive changes in their lives. Attend apartment viewings and other appointments with clients. Conduct independent home visits once clients are placed in housing. To administer financial assistance to stabilize housing Request financial assistance for security deposits and rental assistance as appropriate. Conduct re-certification for households requiring more than three months of financial assistance.

Work with property owners and Housing Location staff to obtain all necessary documentation to support financial assistance requests. Maintain professional relationships with all staff and residents. Accurately and promptly document all activities, services, and outcomes in all required formats ClientTrack, HMIS, paper file, etc. To maintain complete and accurate program documentation Maintain complete and orderly client files, including proof of homelessness and proof of income.

Complete income recertification and other documentation needed for various funding sources. Ensure that all resident assessments, services, and outcomes are promptly and thoroughly documented in ClientTrack, HMIS, and other tracking systems as required. Ensure that all resident information is updated at least quarterly. Obtain supporting documentation for all outcomes reported including proof of permanent housing, proof of income, documentation of life skills obtained, etc.

Maintain current release of information documentation for all collateral contacts for each resident. Assist Program Coordinator in preparing for all program audits. Utilize a strengths-based approach, harm reduction strategies, and motivational interviewing strategies to support resident progress toward individual goals and positive change. Provide assertive engagement when youth are disengaged from programming or at risk of discharge. When appropriate, connect Partner with youth and staff in creating opportunities for youth leadership and community building within Solid Ground and the community.

Participating in on- and off-site staff trainings, workshops and outreach events as needed. Collaborating in a team approach to develop new programs. Participate in on-call rotation to support all youth programs overnight and on weekends. Perform other duties as needed. Plan a monthly schedule for events, activities, and food menu. Deliver twice weekly psychoeducational programming to youth during drop-in hours.

Collaborate with other internal programs to provide assistance to youth Goal Two: To provide outreach services to homeless youth ages Conduct targeted and organized outreach to youth serving entities such as schools, park districts, community based organizations, and law enforcement agencies in order to recruit youth who could benefit from our services.

Conduct street outreach to locations where youth experiencing homelessness congregate both formally and informally. Deliver presentations about the services of Casa Corazon to both small and large groups of people in an engaging and informative manner. Maintain an organized outcomes based accounting of outreach efforts and contacts with homeless youth and necessary follow-up.

To work in team approach to maintain a positive and safe milieu at Casa Corazon Observes environment to ensure safety of youth and staff by responding to emergencies, building repairs, safety hazards, crisis and incidents.

Maintains a safe and healthy environment for staff and Youth through shared responsibilities of cleaning. Maintain on-going dialogue with staff around areas of youth successes and challenges. Follow up with any incident in a timely manner utilizing a strength base approach. Administrative Responsibilities Will maintain organization and inventory of all toiletries, food, bus cards and other essential items.

Teach and implement the declaration of non-violence at Casa Corazon. Intermittently gather and complete data entry for various grant reports and development purposes. Work in conjunction with Program Manager and Drop in Coordinator to meet program outcomes.

Provide and coordinate life skills workshops to youth focused on personal hygiene, sexuality, financial literacy, household management, conflict resolution, anger management, and other areas as needed Implement established structure of support and accountability within the program to allow youth to fully develop in all areas of their life To implement standards of cleanliness and maintenance by performing duties and requesting repairs Leading residents in the upkeep of the Low Threshold Program and working with residents to ensure that necessary tasks are completed Cleaning and sanitizing areas as needed to ensure standard of cleanliness and sanitation is kept up at all times In addition to the responsibilities listed above, all La Casa Norte employees are responsible for contributing to the overall success of La Casa Norte by: Greet and direct all Howard Brown Health visitors with behaviors that balance professionalism and create customer loyalty.

Notify Howard Brown staff of all visitors, take messages and provide basic referral information within the scope of knowledge and training. Notify interviewing manager of applicant upon completion of application, as needed. Input demographic information, billing information and insurance into medical management system. Conduct financial assessments for designated patients as described in the policy and procedure. Document covered benefits on the insurance verification form. Educate clients regarding all financial and insurance benefits, government funds, grant, and drug reimbursement programs.

Ensure all requirements are met. Assist Patient Accounts Manager in billing and collection of patient accounts. Collects co-payments Checks ID and Insurance card for all visits Ensure patient compliance with agreed payment plan.

Assist in development and revision of billing policies and procedures. Participate in agency wide quality assurance and improvement as needed. Check-in and check-out all patients using the time clock within medical management system.

Maintain balanced cash banks for all research studies stipends and cash drawer. Input third party reimbursements Medicaid, Medicare, and Insurance into medical management system via batch processing as needed.

Assists patients with identifying appropriate pharmaceutical industry patient assistance programs for medications prescribed by a Howard Brown Health PCP.

Collaborates with patient to prepare and submit applications as appropriate. Consults with potential clients seeking medical services in order to identify most suitable setting for their individual healthcare needs. Participates as a team member with Medical Services staff, attending Medical Services meetings and continuing education in-services.

Participate in emergency preparedness plan as dictated by the policy and procedures. Performing other duties as assigned. Some Saturdays and evening hours required. Space is limited and must be shared for most appropriate usage.

Reports patient observations to the responsible provider or staff nurse. Performs lab procedures as assigned, collects, labels, and processes all specimens including urine, stool, sputum, and blood as required.

Provides patient with appropriate requisition, information and instruction for ancillary tests and procedures ordered by physicians. Stocks inventory and ensure cleanliness of examination rooms, equipment, and work area. Reports medical and office supply inventory needs to supervisor.

Prepares specimens for shipment to external lab ensuring that specimens are ready and picked up by courier in accordance with designated courier schedule. Ensures that specimens are processed in accordance with established Howard Brown research protocol. Participates in quality assurance process via data collection, evaluation of results, and implementation of recommendations.

Participates in research project participant recruitment as directed. Performs data entry into Centricity as needed i. Performs medical reception functions. Mandatory attendance of department meetings. Performs other duties as directed. Supervise and ensure safety of the milieu Communicate in a timely and accurate fashion within the program i.
